Título: CLINICOPATHOLOGICAL ANALYSIS OF ONCOCYTIC MUCOEPIDERMOID CARCINOMA: A SYSTEMATIC REVIEW

Resumo: OBJECTIVE: To characterize the clinicopathological, immunohistochemical, and molecular features of oncocytic mucoepidermoid carcinoma (OMEC) of the salivary glands. STUDY DESIGN: Systematic review conducted according to PRISMA guidelines and registered in PROSPERO (CRD420251040512). Case reports and case series published in English were included. The search was performed in electronic databases and grey literature. Data were analyzed descriptively. RESULTS: A total of 33 studies comprising 97 OMEC cases were included. A slight female predominance (51.55%) and a wide age range (9-84 years; mean age: 54.8 years) were observed. The parotid gland was the most frequently affected site (76.29%). Clinically, tumors commonly presented as asymptomatic nodular masses. Histologically, there was a predominance of oncocytic cells, often associated with mucous and intermediate cells. Most cases were classified as low grade. Immunohistochemical findings were non-specific, with frequent expression of p63, CK5/6, and CK7. MAML2 rearrangement was consistently identified across cases, with very high detection rates depending on the method used. Surgical treatment was the main approach, with low recurrence and metastasis rates. CONCLUSION: OMEC typically shows indolent behavior and a favorable prognosis and reinforces its classification as a predominantly low-grade morphological variant of conventional mucoepidermoid carcinoma, supported by the frequent presence of MAML2 rearrangements.
