Título: Monophasic Synovial Sarcoma: Case Report

Resumo: A 22-year-old female was referred to the stomatology department complaining of swelling in the cheek region lasting for one year. Physical examination revealed a hardened nodule with irregular edges in the left buccal mucosa region, measuring approximately 0.5 cm, and the presence of granular secretion upon palpation. An incisional biopsy was performed. Histological analysis of the H&E-stained slides revealed cells without a specific arrangement, pleomorphic nuclei, scant cytoplasm, and evident mitotic activity. The diagnostic report was compatible with an undifferentiated malignant neoplasm. The patient was referred to a head and neck surgeon and underwent a left partial hemimandibulectomy. Microscopic examination with immunohistochemical analysis revealed moderate nuclear pleomorphism, with perineural, bone, and soft tissue invasion. Immunohistochemical findings showed 70% positivity for Ki-67, diffuse positivity for vimentin, CD99, and TLE1, and preserved nuclear expression of histone H3K27me3 in neoplastic cells. Diagnostic tests supported the diagnosis of monophasic synovial sarcoma.
