Título: PEDIATRIC DESMOPLASTIC FIBROMA OF THE MANDIBLE: A RARE CASE WITH COMPREHENSIVE HISTOPATHOLOGICAL AND IMMUNOHISTOCHEMICAL

Resumo: Desmoplastic fibroma is a rare benign intraosseous tumor with locally aggressive potential. A 3-year-old female presented with an asymptomatic swelling in the left posterior mandibular region with approximately one year of evolution. Clinical and radiographic examination revealed an expansive multilocular radiolucent lesion involving the left mandibular body, angle, and ramus, with cortical thinning and focal cortical perforation. Incisional biopsy showed densely collagenized fibrocellular connective tissue, composed of thick, wavy collagen bundles in whorled and fascicular patterns, with relative hypocellularity and bland spindle-shaped fibroblasts in a mature collagenous matrix. Immunohistochemistry demonstrated strong positivity for vimentin, nuclear positivity for β-catenin, a low Ki-67 proliferative index, and negativity for cytokeratins, S100, CD34, and α-SMA, confirming the diagnosis of desmoplastic fibroma of the mandible. The patient was referred for surgical resection and remains under follow-up.
