Título: Advanced Oral Squamous Cell Carcinoma in a Homeless Patient: Diagnostic Delay and Social Vulnerability

Resumo: Oral squamous cell carcinoma (OSCC) is an aggressive malignant neoplasm frequently associated with delayed diagnosis and poor prognosis in Brazil. A 56-year-old homeless man presented an extensive intraoral exophytic and vegetative lesion, with exteriorization to the extraoral region, showing an irregular surface, ulcerated areas, and indurated borders involving the lower third of the face. The patient had no family support and his verbal communication was limited, which hindered the collection of a detailed clinical history. Histopathological examination of the incisional biopsy revealed the proliferation of malignant squamous epithelial cells in the form of islands within the underlying stroma. These cells exhibited hyperchromatism, marked pleomorphism, mitotic figures and foci of keratinization, confirming the diagnosis of OSCC. The patient was referred for oncologic care, but refused the proposed treatment and did not return for consultations. This case highlights the impact of social vulnerability in delayed diagnosis, poor prognosis, and continuity of care.
