Título: LATE DIAGNOSIS OF ORAL LENTIGINOUS MELANOMA: A WARNING FOR CLINICAL PRACTICE

Resumo: Melanoma is a malignant neoplasm of melanocytic origin that is rare in the oral mucosa, accounting for approximately 0.5% of malignant tumors; however, when present, it tends to exhibit more aggressive behavior than its cutaneous forms. A 78-year-old male with no comorbidities presented with an extensive, symptomatic exophytic growth in maxilla. The lesion extended from the edentulous alveolar ridge to the hard palate and vestibular mucosa, exhibiting ill-defined margins, an irregular surface, poorly demarcated borders, and a multinodular appearance with vegetative areas. Its coloration was heterogeneous, predominantly blackish, with associated erythematous areas and foci of ulceration. Histopathological analysis of the incisional biopsy revealed proliferation of atypical melanocytes in the basal layer of the epithelium, along with diffuse and asymmetric invasion of spindle-shaped melanocytes into the underlying connective tissue. Immunohistochemical analysis showed positivity for HMB-45, S-100, and Ki-67, confirming the diagnosis of melanoma. The patient was referred for specialized oncologic treatment.
