Resumo: Multiple myeloma is a hematologic malignancy characterized by clonal proliferation of plasma cells in the bone marrow. Despite recent therapeutic advances, many patients develop relapsed or refractory disease, requiring novel treatment approaches. Talquetamab, a bispecific antibody targeting GPRC5D and CD3, has demonstrated promising efficacy; however, its mechanism of action is frequently associated with oral toxicities that may compromise nutritional status and quality of life. This case series evaluated oral alterations in eleven patients with multiple myeloma treated with talquetamab at a Brazilian cancer center between January 2024 and October 2025. The mean age was 64.6 years, with a male-to-female ratio of 6:5. Xerostomia occurred in all patients, requiring therapy with bethanechol. Dysgeusia was reported in 72.7%, and oral candidiasis developed in 18.2%. A mean weight loss of 9.9% was observed. These findings highlight the importance of early multidisciplinary management to mitigate oral toxicities and improve patient outcomes.
