Resumo: A 46-year-old female patient with type II diabetes mellitus presented with a four-month progressive swelling in the left mandibular region. Clinical and imaging findings revealed an expansive lesion in the mandibular body, confirmed as odontogenic myxoma by incisional biopsy. Treatment involved resection with safety margins, followed by reconstruction using an autologous iliac crest bone graft. The postoperative course was uneventful, however, after 15 days, bone exposure occurred at the proximal stump, managed with local abrasion. Seven days later, infection developed, controlled with antibiotic therapy amid laboratory evidence of glycemic dyscontrol. At 45 days postoperatively, debridement and removal of devitalized bone were performed. With clinical stabilization and glycemic control, the patient showed no infectious signs but exhibited partial graft resorption. This case highlights the impact of metabolic control on bone integration and underscores the need for multidisciplinary management in reconstructive planning.
