Resumo: Objective
To map available evidence on cancer stem cell (CSC) biomarkers associated with oral field cancerization in oral potentially malignant disorders (OPMD) and oral squamous cell carcinoma (OSCC), and to identify their roles in malignant transformation, tumor progression, and prognosis.
Study design
A scoping review was conducted through a structured search of PubMed database for studies published between 2015 and 2025. Studies evaluating CSC biomarker expression in tissue samples from patients with OPMD, with or without epithelial dysplasia, and OSCC were included. Graphs were created using data to identify biomarker expression patterns associated with field cancerization events.
Results
ALDH1 and Bmi-1 were frequently associated with increased risk of malignant transformation in OPMD. CD44, E-cadherin, and vimentin showed relevance in tumor progression and recurrence. Co-expression of CD44 and CD31 suggested interaction between CSC pathways and tumor microenvironment. ABCG2 and podoplanin were overexpressed in OPMD, indicating early involvement. OCT4/3, SOX2, and NANOG were linked to tumorigenesis and poorer prognosis, with SOX2 also showing prognostic value.
Conclusions
Available evidence suggests CSC biomarkers contribute to understanding early molecular events in field cancerization and malignant transformation risk. Heterogeneity across studies highlights the need for standardized research to define prognostic value and guide personalized surveillance strategies.
