Resumo: Simple bone cyst (SBC) is a pseudocystic intraosseous lesion of uncertain etiology, often diagnosed incidentally. This case series reports three patients, two males (15 and 17years old) and one female (11 years old), presenting with asymptomatic, unilocular radiolucent lesions in the mandibular premolar and molar region, exhibiting a scalloped pattern on imaging. Lesions were identified as incidental radiographic findings. Aspiration yielded serosanguineous fluid and surgical exploration revealed an empty cavity with scant material, which was submitted for histopathological analysis and showed blood clot, vascular structures, and leukocytes. The combined radiographic, surgical, and histopathological findings supported the diagnosis of SBC. All patients were managed conservatively with surgical exploration and followed clinically and radiographically for over 18 months, demonstrating progressive bone regeneration without recurrence. These findings reinforce the importance of correlating clinical, imaging, and intraoperative features for accurate diagnosis and appropriate management of SBC.
