Título: OROPHARYNGEAL BURKITT LYMPHOMA AS A MANIFESTATION OF HIV INFECTION

Resumo: Burkitt lymphoma (BL) rarely presents in the oral cavity, and oropharyngeal involvement is uncommon. A 30-year-old male presented with a 5 cm sessile tumor involving the oropharynx and soft palate, with a crateriform surface and telangiectatic areas. The patient was HIV+, with a viral load of 32,900 copies/mL, CD4 count of 242 cells/µL, CD8 count of 920 cells/µL, and CD4/CD8 ratio of 0.26. Incisional biopsy revealed a lymphoproliferative lesion with a starry-sky pattern, composed of monomorphic medium- to large-sized cells, frequent mitotic figures, and apoptotic bodies. Immunohistochemical analysis showed CD45, CD20, CD79a, PAX5, CD10, BCL6, and Ki-67 (100%) positivity, and negativity for p40, CK34βE12, and CD3. A diagnosis of BL was made. The patient received antiretroviral therapy (TLD regimen) and 3 cycles of chemotherapy (R-CODOX-M), achieving complete remission of the lesion. BL in the oral cavity may represent a primary manifestation or indicate disease progression in HIV+ patients.
