Resumo: Objective: Burning Mouth Syndrome (BMS) diagnosis remains based on exclusion and lacks objective biomarkers, thus this study aimed to investigate whether saliva may serve as an auxiliary diagnostic tool for BMS using Fourier-transform infrared (FT-IR) spectroscopy. Study design: Saliva samples from 26 patients with BMS and 14 matched controls were analyzed by FT-IR spectroscopy to compare salivary biochemical profiles between groups. Results: Patients with BMS showed a distinct and reproducible salivary molecular profile compared with controls. FT-IR analysis discriminated BMS cases with 80% accuracy and high sensitivity, identifying biochemical differences involving proteins, lipids, nucleic acids, and carbohydrates. Conclusion: Saliva from patients with BMS exhibits measurable biochemical alterations detectable by FT-IR spectroscopy which may represent a promising noninvasive adjunctive tool to support diagnosis and improve clinical phenotyping.
