Resumo: An 18-year-old white male presented with asymptomatic intraoral drainage. Clinical examination identified purulent discharge from the anterior palatal rugae upon palpation, with positive pulp vitality in teeth 13 to 23. Cone beam computed tomography revealed a hypodense image centered in the incisive canal region, closely related to central anterior incisors, with well-defined borders, internal hyperdense foci, and communication with both the oral and nasal cavities. These radiographic features suggested a differential diagnosis of calcifying odontogenic cyst and adenomatoid odontogenic tumor. The lesion was surgically enucleated with thorough curettage. Histopathological examination demonstrated a cavity lined by non-keratinized stratified squamous epithelium with ciliated areas, acute inflammation in the capsule, prominent Actinomyces colonies and mineralized structures. A diagnosis of nasopalatine duct cyst infected with actinomycosis was established. The patient remains under follow-up with no evidence of recurrence or further infection. Adjacent teeth remain vital and are under endodontic surveillance.
