Resumo: Objective: To report preliminary findings from the implementation of an Oral Cancer Screening and Matrix Support Program in a Brazilian health region, serving as the basis for a pragmatic clinical trial.
Study Design: The Oeste II Health Region (Goiás State) includes 13 municipalities, including one with high oral cancer hospitalization and mortality rates. A structured care pathway was established with matrix support from a Federal University oral medicine center (teleconsulting, histopathological examinations, complex case management). The Program involved: 1) capacity building for primary care professionals in early oral cancer detection, and 2) deployment of a digital platform for risk population monitoring.
Results: 566 professionals attended in-person training (30 days) plus a 40-hour distance learning course. Over four months (November 2025-February 2026), 666 high-risk individuals were screened, 42 referred to secondary care, and 15 biopsies performed. Histopathology revealed: 3 malignant lesions, 4 oral potentially malignant disorders (OPMD), 5 benign lesions, and 3 inflammatory conditions. This four-month targeted screening detected the expected annual lesion incidence for the general population. Pragmatic trial data analysis is ongoing.
Conclusion: Implementing a structured care pathway focused on high-risk groups, combined with primary care training, significantly accelerated the oral cancer and OPMD detection. Cost-effectiveness studies are needed.
