Título: Using Immunohistochemical Staining to Aid in the Diagnosis of Oral Mucous Membrane Pemphigoid and Pemphigus Vulgaris

Resumo: Introduction: Oral mucous membrane pemphigoid (MMP) and pemphigus vulgaris (PV) can be challenging diagnoses even with direct immunofluorescence (DIF). This study investigated immunohistochemical (IHC) staining as an alternative diagnosis method. Materials and Methods: After IRB approval, a retrospective search yielded ten cases each of DIF positive MMP and PV, and ten cases of DIF negative hyperkeratosis, which served as the control. IHC staining was performed using C3d and IgG4 antibodies and interpreted via a consensus agreement between two pathologists. Specificity and sensitivity were calculated for IHC and DIF against the control. Results: MMP C3d IHC demonstrated lower sensitivity than DIF C3, and IgG4 IHC showed lower sensitivity than DIF IgG. For PV, C3d IHC demonstrated higher sensitivity than DIF C3, and lgG4 IHC showed lower sensitivity than DIF IgG. All tests displayed 100% specificity for MMP and PV compared to control specimens. Conclusions: Overall, IHC stains were less sensitive than DIF for PV and MMP, emphasizing DIF as the gold standard for diagnosis. However, since all IHC tests showed equal specificity to DIF, it may be concluded that in cases where DIF is not feasible, IHC may be an alternative testing method.
