Título: MANDIBULAR SWELLING AS A RARE MANIFESTATION OF TIBIAL OSTEOSARCOMA METASTASIS: A CASE REPORT

Resumo: A 57-year-old male patient with a history of osteoblastic osteosarcoma of the left tibia was referred due to swelling in the right mandibular region. Extraoral examination revealed mild swelling, and intraoral evaluation showed volumetric increase in the inferior vestibular fornix. Computed tomography demonstrated a hypodense lesion measuring 5.2 × 2.9 cm, with bone expansion and cortical disruption. Incisional biopsy revealed a hypercellular neoplasm composed of poorly differentiated fusiform and epithelioid cells with marked atypia and osteoid-like matrix. Immunohistochemistry was positive for INI-1, Ki-67, p16, and p53. These findings supported the diagnosis of mandibular metastasis of osteosarcoma. The patient initially underwent chemotherapy with doxorubicin, with disease progression. After metastasis diagnosis, second-line chemotherapy with etoposide and ifosfamide was initiated, along with palliative radiotherapy. The patient presented with progressive local tumor-related complications and died in April 2026.
