Título: THE RARE ASSOCIATION BETWEEN MULTIPLE ESCLEROSIS AND PEMPHIGUS VULGARIS: A CASE REPORT

Resumo: A 37-year-old man with a history of multiple sclerosis in treatment with quetiapine and dimethyl fumarate presented to our service complaining of a metallic taste, great weight loss, and painful oral lesions. Previous diagnosis in another service were aphthous ulcer and candidiasis. He was treated with prednisone, acyclovir, antifungals, and eye drops without success. Extraoral examination revealed dry nasal mucosae, cutaneous red patches, and ocular erythema. Intraorally, multiple red lesions involving the tongue were observed, with hemorrhagic ulcers on the buccal mucosa, retromolar trigone, gingiva, and buccal vestibule. Incisional biopsy was performed in the buccal vestibule. Microscopic examination with hematoxylin/eosin and direct immunofluorescence established the diagnosis of pemphigus vulgaris. The patient was referred to dermatologist and was treated with prednisolone. Four months later, he presented recurrence and began additional therapy with rituximab and prednisone (higher dose). The patient is currently without lesions and returned to his normal weight.
