Resumo: Objective: To analyze the clinicopathological characteristics of patients diagnosed with acinic cell carcinoma (AciCC) of the salivary glands. Study Design: A retrospective clinicopathological study was conducted on a cohort of 30 patients, utilizing medical records and histopathological data. Results: According to the available data, the mean age was 46.3 years (range, 9-71), with a slight female predominance (52%). The parotid gland was the primary site in 93.3% of cases. Clinical staging (n=17) ranged from stage I (13.3%) to stage IV (10.0%). Surgical treatment was performed in all cases with available data (n=17). Surgical margins were free or involved in 30% of cases each. Most tumors were classified as low grade (86.6%), 6.7% as intermediate grade, and 6.7% showed high-grade transformation. Lymphovascular invasion was identified in 6.7% of cases, and no perineural invasion was observed. Follow-up (1-131 months; n=17) demonstrated that 82.4% of patients were disease-free, whereas one case (3.3%) developed late distant metastasis. Conclusion: AciCC typically behaves as a low-grade malignancy with a favorable prognosis. However, some low-grade cases exhibited late metastasis and lymphovascular invasion, suggesting that clinical behavior can be unpredictable and requires long-term monitoring regardless of histological grade.
