Resumo: Introduction: Hyper-IgE syndrome (HIES) is a rare primary immunodeficiency (~1/million), characterized by elevated serum IgE levels, skin eczema, and increased susceptibility to infections. It is associated with mutations in STAT3 and DOCK8. Oral manifestations are infrequently reported.

Case Report: A 23 years old male, diagnosed with HIES in childhood, with a history of asthma, dermatitis, pneumonia, and cutaneous abscesses. Presented a prominent forehead, broad nasal bridge, fleshy nasal tip and multiple oral lesions, including a papule on the lower lip vermilion, a nodule on the lower labial mucosa, and a multilobulated nodule on the right lateral border of the tongue. Excisional biopsies were performed. Histopathological analysis revealed verruca vulgaris, chronic inflammatory process with eosinophils, and traumatic ulcerative granuloma with stromal eosinophilia, respectively. Clinical outcome was favorable after surgical excision. Recognition of oral manifestations supports timely prevention, diagnosis, and follow-up, as management is primarily supportive and multidisciplinary.
