Resumo: Objective: To evaluate radiographic features of the jaws in patients exposed to antiresorptive medications and their relationship with medication-related osteonecrosis of the jaws (MRONJ). Study design: A total of 166 participants were included and allocated into two groups: MRONJ (n=27) and non-MRONJ (n=139). Imaging assessments were performed using panoramic radiography (PAN) and cone beam computed tomography (CBCT) to identify radiographic features related to MRONJ. Results: On PAN, the MRONJ group exhibited a higher prevalence of osteolysis, persistent alveolar socket, bone sequestration, mandibular canal involvement, periosteal reaction, and sinus involvement. A sclerotic trabecular bone pattern was observed in both groups. CBCT analysis revealed higher frequencies of sclerotic trabecular bone pattern, mandibular canal involvement, and bone sequestration among patients with MRONJ. Composite radiographic index modified (CRIm) scores were higher in the MRONJ group for both imaging modalities and showed a positive correlation with disease staging. Conclusion: Radiographic imaging plays a crucial role in the early diagnosis of MRONJ. The CRIm appears to be a reliable tool for disease staging and monitoring.
