Resumo: A 48-year-old female presented with asymptomatic swelling in the right maxillary region for more than 2 years. Clinical examination revealed bicortical bone expansion extending from the premolar region to the maxillary tuberosity. Panoramic radiography showed a mixed radiolucent-radiopaque lesion with scattered radiopaque foci extending from the periapical region to the inferior orbital rim, with displacement of the maxillary sinus toward the midline. Histopathology revealed a neoplasm composed of cords and nests of polyhedral epithelial cells with prominent intercellular bridges and mild nuclear pleomorphism, associated with extracellular deposits of eosinophilic, amyloid-like material and concentric basophilic calcifications. The diagnosis was calcifying epithelial odontogenic tumor (CEOT). The lesion was excised, with no recurrence after 9 months. CEOT is a rare benign odontogenic epithelial neoplasm characterized by amyloid-like deposits and Liesegang ring calcifications. This case highlights an extensive lesion in the maxilla, an uncommon site, with marked bone expansion and displacement of adjacent structures.
