Resumo: The therm Lichenoid Leukoplakia has been passionately discussed over the years, with a predominant opinion that the term should be avoided. We introduce a case involving a 58-year-old non-smoking, non-drinking female with chronic alcohol-based mouthwash use and a long-standing ill-fitting maxillary denture with multiple painless leukoplakic lesions present in the alveolar maxillary ridge, buccal mucosa in the upper posterior region, ipsilateral retromolar trigon and lower third molar gingiva. Clinically these lesions were extensive, non-scrapable white plaques ranging from smooth to rough, with cardboard-like texture. Multiple biopsies were performed ranging from cytopathologic to histopathologic with immunohistochemical findings. Viral cytopathologic changes such as koilocytosis, Cowdry inclusions, EBV, P16, P53 positivity confirms the strong viral influence on the lesional development. Histopathological diagnoses always found moderate to severe dysplasia despite discontinuation of mouthwash and prosthetic use. One of the biopsies showed more than one recurrence and the patient remains under close follow-up to date.
