Título: FRACTAL DIMENSION AND PIXEL ANALYSIS OF MANDIBULAR BONE IN OSTEORADIONECROSIS: A COMPARATIVE AND METHODOLOGICAL STUDY

Resumo: Objective: To evaluate fractal dimension (FD), lacunarity (Lac), and pixel intensity (mean [M] and standard deviation [SD]) in mandibular bone of patients with osteoradionecrosis (ORN), compared with irradiated (IR) and control (C) groups, and to assess agreement between two FD methods. Study design: This retrospective cross-sectional study included 69 panoramic radiographs (ORN=11, IR=27, C=31). Two bilateral cortical regions of interest (50×50 pixels) and two bilateral trabecular bone regions (100×100 pixels) were selected for each patient. FD was calculated using FracLac (FD.f) and a binarization/skeletonization method (FD.c). Lac, M, and SD were obtained from texture and histogram analyses. Kruskal-Wallis with Dunns post-hoc test was used for intergroup comparisons, and Bland-Altman assessed agreement between FD methods on trabecular ROI. Results: Cortical FD.f was higher in ORN compared with IR and C (p<0.001). Cortical Lac was lower in ORN than IR (p<0.001), while SD was higher in ORN than C (p=0.03). Trabecular FD.f also differed between groups C vs IR and ORN vs IR (p<0.05). Bland-Altman analysis demonstrated agreement between FD.f and FD.c. Conclusion: Mandibular bone in ORN exhibits altered cortical and trabecular complexity compared with irradiated and control groups. The two FD methods showed good agreement despite differences in image processing approaches.
