Título: Mandibular Involvement as an Initial Manifestation of Multiple Myeloma: A Case Report

Resumo: Multiple Myeloma (MM) is a malignant plasma cell disorder that primarily affects the axial skeleton but may also involve the mandible. Oral manifestations may represent early signs of systemic disease or disease progression, often mimicking odontogenic conditions, highlighting the importance of differential diagnosis. A 66-year-old male was referred to the Oral Medicine Department for evaluation of a lesion observed on panoramic radiography. His medical history included a prior diagnosis of solitary bone plasmocytoma in the vertebra. Intraoral examination revealed no clinical abnormalities. Panoramic radiography showed multiple osteolytic lesions in the right mandible and maxilla. Biopsy confirmed plasma cell infiltration consistent with MM, with kappa light chain restriction and CD138/CD56 positivity. This case highlights the importance of oral evaluation in systemic disease detection.
