Resumo: A 52-year-old female patient presented with numbness affecting the left lower third of the face and a burning sensation in the right hemilingual region following mandibular third molar extraction. Neurosensory assessment was performed by comparing the affected areas with their contralateral counterparts, leading to the diagnosis of inferior alveolar nerve paresthesia. The therapeutic protocol consisted of photobiomodulation using a defocused high-power superpulsed laser (Gemini Evo), with intraoral irradiation delivered through a 7-mm tip at 0.3 W, 0.38 cm², and 6 J per point across 25 sites, and extraoral irradiation through a 25-mm tip at 1 W, 4.91 cm², and 20 J per point across 10 sites along the anatomical course of the affected nerve. A total of 20 sessions were performed, three times weekly. Significant improvement in paresthesia was observed, accompanied by symptom reduction and progressive functional recovery.
