Título: aPDT FOR ORAL CANDIDIASIS IN HEAD AND NECK RADIOTHERAPY PATIENTS: CLINICAL AND MICROBIOLOGICAL PILOT STUDY

Resumo: Objective: This pilot study aimed to evaluate the clinical and microbiological efficacy of antimicrobial photodynamic therapy (aPDT) in the treatment of oral candidiasis in patients undergoing radiotherapy for head and neck cancer, compared with conventional antifungal therapy using nystatin.
Study design: This is a preliminary prospective, interventional, and controlled clinical study including five patients allocated into two groups: aPDT (n=3) and nystatin (n=2). Participants were followed longitudinally, with clinical and microbiological assessments performed before and after treatment.
Results: Clinically, aPDT showed superior outcomes, with complete lesion resolution in 66.6% of patients and partial response in 33.3%, whereas the nystatin group showed no complete resolution and 50% of cases without improvement. Both therapies reduced fungal load; however, a greater reduction was observed with aPDT (39%) compared to nystatin (30%). Candida albicans was the most prevalent species (55.5%), and high resistance to azoles was observed, while amphotericin B showed the greatest antifungal activity.
Conclusion: Both therapies were safe and effective; however, aPDT demonstrated better clinical performance and greater reduction in fungal load, without inducing antifungal resistance. These findings suggest that aPDT may be a promising adjunctive therapy, although larger studies are needed to confirm its efficacy.
