Título: UNUSUAL LINGUAL ABSCESS FROM A PEQUI SPINE: CASE REPORT

Resumo: A 57-year-old female patient presented with a normochromic symptomatic submucosal
nodule at the left apex of the tongue, with a 15-day history following pequi ingestion.
On clinical examination, a lesion measuring 5 mm with fibroelastic consistency was
observed. Based on the clinical hypothesis of a foreign body reaction, an excisional
biopsy was performed. Histopathological examination of the specimen with
hematoxylin and eosin revealed a fragment of lingual mucosa covered by surface
epithelium and an intense mixed inflammatory infiltrate consisting predominantly of
neutrophils, macrophages and lymphocytes surrounding a foreign body particle, as well
as adjacent bacterial colonies. Under polarized light microscopy, the foreign particle
demonstrated birefringence, highlighting fine structures compatible with a spine from
the Brazilian fruit pequi. Clinicopathological correlation allowed the diagnosis of a
lingual abscess due to a foreign body identified as a pequi spine. On follow-up, the
patient showed satisfactory evolution, with no recurrence or new complaints.
