Título: POST-TRANSPLANT LYMPHOPROLIFERATIVE DISORDER AFFECTING THE ORAL CAVITY OF A HEART TRANSPLANT CHILD: A CASE REPORT

Resumo: An 8-year-old female patient who underwent bicaval orthotopic heart transplantation at age 3, receiving sirolimus and tacrolimus, presented to the hospital with oral pain and difficulty eating. Intraoral examination revealed ulcerated lesions on the right lateral border of the tongue and left buccal mucosa with approximately 10 days of evolution. An incisional biopsy of the lesions was performed under sedation. While awaiting results, photobiomodulation sessions were instituted (808 nm, 100 mW, 3 J) for pain control, along with mouth rinses using an oral solution containing nystatin, vitamin B complex, and 2% lidocaine. Histological sections revealed dense infiltration of mononuclear inflammatory cells with positive hybridization for Epstein-Barr virus detected as dark blue dots, and scattered CD3⁺, CD20⁺, CD4⁺, CD8⁺, CD38+ cells, confirming polymorphic post-transplant lymphoproliferative disorder. The patient was referred for pediatric oncology follow-up and treated with rituximab. After one year, complete healing was observed, with no recurrence under multidisciplinary follow-up.
