Título: HISTOMORPHOMETRIC EVALUATION OF DIODE LASER-INDUCED THERMAL DAMAGE IN ORAL SOFT TISSUE LESIONS

Resumo: Objective: To quantify thermal alterations in collagen fibers of oral soft tissue lesions excised with a 980-nm diode laser and compare them with scalpel excision. Study Design: Forty-five lesions were excised using a 980-nm diode laser (2.5 W) in pulsed or continuous mode, or with a scalpel (control). Samples were stained with Masson's trichrome and analyzed by digital histomorphometry using color deconvolution and area measurement. Statistical analysis included the Kruskal-Wallis test with Bonferroni post hoc correction (p < 0.05). Results: Laser-excised lesions showed a significantly higher percentage of altered collagen compared to controls (0.73% ± 0.39), both pulsed (16.46% ± 12.7) and continuous modes (20.49% ± 11.74) (p = 0.00094). No difference was found between emission modes (p = 0.850); however, both differed significantly from the scalpel group (p < 0.01). Total energy was positively associated with the percentage of altered collagen (β = 0.014; R² = 0.125; p = 0.017) and the distribution of abnormal collagen was diffuse and extended beyond the site of laser application. Conclusion: Diode lasers induce significant thermal collagen alterations in oral soft tissue lesions regardless of emission mode, and Masson's trichrome staining is a useful method to distinguish thermally altered from normal tissue.
