Resumo: A 62-year-old Caucasian female with a history of smoking presented with a chief complaint of a painful palatal lesion for one year. Intraoral examination revealed an ulcerated swelling on the hard palate. Cervical lymphadenopathy was observed. An incisional biopsy was performed, and histopathological analysis revealed glandular neoplastic cells arranged in tubules and ducts lined by a double-layered epithelium, forming lumina containing hyalinized and mucinous material. No evidence of perineural invasion was identified. The overlying epithelium exhibited cellular atypia and dysplastic changes, with invasion into the connective tissue. The histopathological diagnosis was adenoid cystic carcinoma with areas consistent with superficially invasive squamous cell carcinoma. The patient was referred for oncologic evaluation.
