Título: CENTRAL OSSIFYING FIBROMA: TWO CASE REPORTS HIGHLIGHTING CLINICAL VARIABILITY

Resumo: We report two cases of central ossifying fibroma (COF) with distinct clinical presentations. The first case involved a 40-year-old female presenting with a lesion in the anterior mandible, associated with pain on palpation and tooth displacement. Radiographic examination revealed a well-defined radiolucent lesion containing irregular radiopaque foci, associated with vital teeth. The second case involved an 11-year-old boy with asymptomatic swelling in the posterior mandible, associated with an impacted tooth. Imaging demonstrated a mixed lesion with cortical expansion and thinning. Initial diagnostic hypotheses included calcifying odontogenic cyst, primordial odontogenic tumor, and juvenile ossifying fibroma. An excisional biopsy with curettage was performed in the first case, also associated with a simple bone cyst, and an incisional biopsy in the second. Histopathological analysis revealed a highly cellular fibrous stroma with spindle-shaped cells, trabecular bone, and cementum-like material, with osteoblastic rimming and osteoclasts, confirming COF. Patients remain under follow-up, with favorable outcomes.
