Título: HPV-UNRELATED ORAL SQUAMOUS CELL CARCINOMA IN YOUNG PATIENTS: A CLINICOPATHOLOGICAL ANALYSIS OF 60 CASES

Resumo: Objective: To investigate the histopathological characteristics of HPV-unrelated oral squamous cell carcinoma (OSCC) arising in young patients.
Study Design: This retrospective observational study comprised cases retrieved from the archives of two oral pathology services in Piracicaba, Brazil, and Toronto, Canada. All cases diagnosed as HPV-negative OSCC in patients aged 40 years or younger were included. Histopathological slides were reviewed, digitized, and independently evaluated by two oral pathologists according to Broders' and Bryne's grading systems.
Results: A total of 60 cases were examined. Broders grade II, consistent with moderately differentiated carcinoma, was the predominant category (50.0%). The conventional histologic subtype predominated (95.0%). Minimal keratinization, corresponding to score 3 (5-20% of tumor cells keratinized), was the most frequent keratinization pattern (31.7%). Nuclear pleomorphism was most commonly classified as score 1, indicating limited nuclear atypia with more than 75% mature cells (35.0%). Pattern of invasion grade 4, defined by marked and widespread cellular dissociation into small groups and/or single cells, was the most common invasive pattern (41.7%). Lymphoplasmacytic infiltration was most often scored as 3, corresponding to slight inflammatory infiltrate (46.7%).
Conclusion: HPV-negative OSCC in young patients was predominantly conventional and moderately differentiated, while frequently exhibiting aggressive invasive features.
