Resumo: Extranodal NK/T-cell lymphoma (ENKTL) is a histopathological subtype of non-Hodgkin lymphoma associated with Epstein-Barr virus (EBV) infection. It is characterized by extensive necrosis, causing tissue destruction and deformities. Thi is a case report of ENKTL in a 40-year-old male patient whose primary clinical manifestation was extensive buccal-nasal communication affecting hard and soft palate, associated with live larvae and pain. The patient underwent surgery for mechanical removal of the larvae and tissues for analysis. Immunohistochemical analysis showed positivity for CD3, KI-67, CD56, PAX-5, CD45, and CD5, and negativity for CD20, BCL-6, and AE1/AE3; an association with EBV was confirmed, consistent with the diagnosis of ENKTL associated with oral myiasis. The treatment combined chemotherapy and radiation therapy. The diagnostic challenge between myiasis and ENKTL was essential to improve patient outcomes, mitigate tissue destruction, and reduce morbidity and mortality. ENKTL should be considered in the differential diagnosis of extensive lesions of the palate.
