Resumo: An 80-year-old male presented with a painful ulcerated lesion on the hard palate persisting for over one year. His medical history included hypertension, diabetes mellitus, and heart failure, all medically controlled. He reported a 30-year smoking history. Intraoral examination revealed a 3.0 × 2.5 cm ulcer with indurated borders and a necrotic center. Computed tomography showed an infiltrative lesion with bone involvement, without regional spread. Diagnostic hypotheses were malignant salivary gland tumor and tertiary syphilis. Incisional biopsy revealed epithelial cells arranged in infiltrative cords, with clear to slightly eosinophilic cytoplasm, mild atypia, and low mitotic activity. The stroma was densely hyalinized, with thick septa and perineural invasion. Immunohistochemistry was positive for CK5/6, p63, and CK7, and negative for S-100 and SOX10, leading to the diagnosis of hyalinizing clear cell carcinoma. The patient underwent surgical resection with an indication for adjuvant radiotherapy and remains under follow-up.
