Resumo: A 63-year-old male presented with a painless nodular lesion in the alveolar mucosa with 1 months of evolution. Clinical examination revealed a violaceous nodular mass with ulcerated areas, while panoramic radiography showed no bone involvement. The initial clinical hypothesis included a reactive lesion and Due to the friable nature of the lesion, complete excision was performed. Histopathological analysis revealed an undifferentiated malignant neoplasm with marked pleomorphism, high mitotic activity, and vascular invasion. The architectural relationship with the surface epithelium suggested a metastatic origin. Immunohistochemical analysis supported the diagnosis of metastatic large-cell neuroendocrine carcinoma of gastroduodenal origin. The patient was referred for systemic evaluation, leading to identification of the primary tumor. Despite treatment, the patient died 3 months later due to disease progression. This case highlights the diagnostic challenge of oral metastases and the importance of histopathological evaluation in their identification.
