Resumo: Milia formation is commonly observed after re-epithelialization of blisters in blistering diseases such as epidermolysis bullosa acquisita (EBA), serving as a useful diagnostic clue to differentiate it from other bullous disorders. While milia frequently occur on the skin, their presence on the oral mucosa is rare. A 54-year-old woman was referred for evaluation of white lesions on the upper lip. She reported that at age 46 she was diagnosed with epidermolysis bullosa acquisita (EBA), confirmed by direct immunofluorescence, and had since experienced recurrent trauma-induced blisters and erosions, predominantly affecting friction-prone areas. Extraoral examination revealed multiple small, firm, whitish, asymptomatic papules on the upper lip vermilion, clinically consistent with milia, superficial keratin-filled cysts presenting as pearly, dome-shaped papules measuring 1-2 mm. No biopsy was performed, as the lesions showed spontaneous partial regression, in line with the benign and transient nature of milia.
