Título: CRANIOFACIAL VERSUS LONG-BONE OSTEOSARCOMAS: A COMPARATIVE ANALYSIS OF CLINICAL, PATHOLOGICAL, AND OUTCOME FEATURES

Resumo: Objective: This study compared demographic, clinical, and outcome characteristics of craniofacial and long-bone osteosarcoma (OS). Study design: A retrospective study reviewed archives from a Brazilian tertiary referral center from 2000 to 2025. Primary central craniofacial OS (n=9) and long-bone OS (n=33) were included. Variables were assessed and groups compared using Mann-Whitney U and Fisher's exact tests. Results: Craniofacial patients had a mean age of 41.8 years, most often in the maxillary sinus (55.6%), with pain as main symptom. All evaluable cases had close or positive margins, local recurrence 42.9%, and late metastasis 44.4%. Long-bone patients had a mean age of 27.6 years, most often in the femur (54.5%). Clear margins occurred in 90.9%, recurrence in 12.1%, and metastasis in 66.7%. Significant differences were found for age (p=0.033) and margin status (p<0.001). Conclusion: The main differences between craniofacial and long-bone OS were patient age and margin status. The higher frequency of positive margins in craniofacial OS likely reflects the anatomical complexity of this region. Although differences in recurrence and metastasis were not statistically significant, craniofacial tumors showed a trend toward higher local recurrence, supporting the need for site-specific therapeutic strategies.
