Resumo: Ameloblastoma is a benign odontogenic neoplasm characterized by slow growth and locally infiltrative behavior, often associated with significant bone destruction and potential functional impairment. A 17-year-old male presented with left posterior mandibular swelling and associated paresthesia for 6 months. Panoramic radiography and computed tomography revealed a large, well-defined, unilocular hypodense lesion extending from the mandibular body to the ipsilateral coronoid process. Based on clinical and imaging findings, the diagnostic hypothesis of ameloblastoma was established. Surgical decompression combined with incisional biopsy confirmed conventional ameloblastoma. After 6 months, significant lesion shrinkage, cortical bone thickening, and bone neoformation were observed, enabling enucleation with peripheral osteotomy under general anesthesia. The patient has been followed clinically and radiographically for 5 years, with no evidence of recurrence and satisfactory bone regeneration. Conservative management may represent a viable therapeutic approach in selected cases, provided that long-term follow-up is ensured
