Título: MALIGNANT MIMICRY IN PEOPLE LIVING WITH HIV: PROGRESSIVE DISSEMINATED HISTOPLASMOSIS SIMULATING ORAL MALIGNANT NEOPLASM

Resumo: A 46-year-old male patient, PLHIV, smoker, alcohol user, and illicit drug user, was admitted to a referral hospital for tropical diseases for the management of immunosuppression and pulmonary tuberculosis, presents for approximately one year with erythematous plaques, with a granulomatous surface and ill-defined margins on the hard palate, extending to the soft palate as well as the left gingival and buccal mucosa. Three months prior, the condition progressed with pain, purulent drainage, and an oroantral communication. Computed tomography revealed a hypodense osteolytic lesion with a "moth-eaten" pattern, cortical destruction, and sinus mucosal thickening, reinforcing the suspicion of malignant neoplasm. An incisional biopsy of the hard palate was performed. Microscopically, an intense lymphohistiocytic infiltrate with multinucleated giant cells was observed, along with yeast-like structures positive on Gomori-Grocott staining, compatible with Histoplasma capsulatum, confirming progressive disseminated histoplasmosis. The patient showed improvement after antifungal therapy and was referred for management of the communication.
