Título: CLINICOPATHOLOGICAL AND IMMUNOHISTOCHEMICAL FINDINGS OF EBV-POSITIVE MUCOCUTANEOUS ULCER IN AN OLDER PATIENT

Resumo: An 85-year-old woman, non-smoker, non-drinker, and on chronic polypharmacy presented with a lesion of unknown duration in the right upper alveolar mucosa extending to the buccal mucosa. Clinically, a necrotic base with elevated and indurated borders was observed. An incisional biopsy was performed under the hypothesis of squamous cell carcinoma. Histopathological examination revealed a fragment of oral mucosa with a large, ulcerated area covered by a fibrinopurulent membrane, a predominantly lymphocytic inflammatory infiltrate in the connective tissue, and atypical cells of variable size, including large binucleated cells resembling Reed-Sternberg cells. Immunohistochemistry showed positivity for CD30, CD20, CD3, CD8, and focal CD4, with a Ki-67 proliferation index of approximately 80%. In situ hybridization for EBV was positive, supporting the diagnosis of EBV-positive mucocutaneous ulcer. Advanced age was the sole identifiable factor associated with immunosuppression. Treatment included broad-spectrum antibiotics and local chlorhexidine hygiene, resulting in complete remission without recurrence during follow-up.
