Resumo: Objective: To evaluate the prevalence of uncollected histopathological reports in a Stomatology service by comparing physical records and electronic system data from an implementation science perspective. Study design: A retrospective descriptive observational study with a quantitative approach was conducted. Biopsies performed between July 2020 and March 2025 were analyzed. Reports classified as uncollected in physical records were identified and subsequently cross-checked with the institutional electronic system. Results: Of the 360 biopsies evaluated, 219 (60.8%) were classified as uncollected in physical records. However, electronic system data showed that 187 (52.05%) patients had documented report retrieval, 159 (44.29%) had no record of retrieval, and only 13 (3.65%) did not return. The mean time without retrieval was 794 days. The mean age was 54.3 years, with a predominance of females (53.9%). Benign lesions accounted for 314 cases (87.2%), while 46 cases (12.8%) were malignant. Conclusion: A significant discrepancy between physical and electronic records was identified, suggesting failures in care pathways and communication. From an implementation science perspective, these findings highlight the need for system integration and effective strategies to ensure continuity of care.
