Título: Drug-Induced Oral Pigmentation Associated with Lenalidomide: A Case Report

Resumo: Lenalidomide is widely prescribed for multiple myeloma and is generally well tolerated. While pigmentary changes affecting the skin and hair have been described, there is no report about oral pigmentation. An 80-year-old melanoderma woman receiving lenalidomide presented with asymptomatic pigmented macules on the dorsal surface of the tongue. Given the atypical presentation, an incisional biopsy was performed. Histopathological examination demonstrated diffuse deposition of brownish granular pigment involving the full thickness of the epithelium and extending to connective tissue components, including collagen fibers, blood vessel walls, neural structures, and skeletal muscle. This distribution pattern appears to be distinct from previously reported cases. Recognition of medication-related oral pigmentation is important in patients undergoing systemic therapy, as it may mimic other conditions. Careful clinical and histopathological correlation is essential to establish the diagnosis and guide appropriate management.
