Resumo: Objective: To investigate the effect of the gender-race-region-generation intersection on mortality from oral and oropharyngeal cancer in Brazil from 2015 to 2024. Study Design: Ecological study based on national data from the Mortality Information System (2015-2024). Deaths were identified using ICD-10 codes for oral and oropharyngeal cancer. Intersectional analyses used sex (male/female), race (White/Black), region, and generation (elderly [≥60 years] and non-elderly [20-59 years]). Absolute numbers and proportions were calculated for each intersection across all Brazilian regions included. Results: During the period, 69,082 deaths were recorded, with marked regional disparities across the country. Mortality disproportionately affected men and individuals aged 60 years or older across regions. Distinct racial patterns were observed: in the North, Northeast, and Midwest, deaths were concentrated among elderly Black men, while in the South and Southeast, elderly White men represented the majority. Oropharyngeal cancer showed the greatest gender disparity, with more than 80% of deaths among men in some regions. Higher mortality among younger individuals was observed for oropharyngeal cancer in the Midwest region.
Conclusion: Mortality is influenced by pronounced racial, gender, and generational inequalities, with regional variation. These findings highlight structural inequalities and reinforce the need for equity-oriented public health strategies.
