Título: NEUROMA OF THE POSTERIOR DORSAL TONGUE: A DIAGNOSTIC CHALLENGE OF A SPINDLE CELL LESION - CASE REPORT

Resumo: A 65-year-old female patient presented with an asymptomaticnodular lesion on the right posterior dorsal tongue, with a one-month history. Clinical examination revealed a submucosalnodule without color changes or evident growth. An incisional biopsy was performed with an initial hypothesis of a mesenchymal or salivary gland neoplasm.
Histopathological analysis showed spindle cells arranged in fascicular and storiform patterns, with oval to elongatednuclei. Immunohistochemistry demonstrated positivity for S100 protein and SOX10, and negativity for actin, desmin, CD34, EMA, and Ki-67, indicating neural origin and lowproliferative activity. A marked postoperative increase in lesion volume led to urgent surgical excision with safetymargins. The diagnosis was confirmed after analysis of thesurgical specimen. This case highlights the importance ofimmunohistochemistry and appropriate clinical management in lesions with unexpected behavior.
