Título: Between stroma and tumor: the role of cancer-associated fibroblasts (CAFs) in malignant salivary neoplasms

Resumo: Objective: To evaluate the presence of CAFs in the tumor microenvironment of malignant salivary gland tumors and their relationship with prognosis. Study design: A scoping review was conducted following PRISMA-ScR guidelines, including observational studies that assessed stromal activation markers and CAFs through immunohistochemistry in relation to prognosis. Results: Mucoepidermoid carcinoma was the most frequently studied tumor overall and among those without myoepithelial differentiation, while adenoid cystic carcinoma predominated among tumors with myoepithelial differentiation. Markers were grouped by biological function, as stromal activation/CAF-related markers, including α-SMA, vimentin, TGF-βR1, and TGF-βRII. Higher stromal α-SMA expression was observed in tumors without myoepithelial differentiation, whereas tumors with myoepithelial differentiation showed α-SMA staining in both stromal and tumor compartments. α-SMA-positive fibroblasts were observed at the tumor-stroma interface with heterogeneous distribution While some studies showed a decrease in myofibroblasts inversely proportional to tumor grade, others demonstrated a higher density of CAFs in aggressive areas. Vimentin analysis revealed coexistence and predominance of myofibroblasts in the stroma. TGF-β1 was expressed in stromal fibroblasts, while TGF-βRII expression was inversely associated with tumor grade. Conclusion: CAF distribution was heterogeneous and inconsistently associated with tumor grade, suggesting dynamic behavior and unclear prognostic impact.
