Título: p53 Status and Genomic Instability in HPV-associated and HPV-independent Oral Epithelial Dysplasia

Resumo: Objectives:
Human papillomavirus-associated oral epithelial dysplasia (HPVOED) is recognized as distinct from conventional oral epithelial dysplasia (non-HPVOED). While direct TP53 mutations frequently drive non-HPVOED, HPVOED likely stems from HPVE6-mediated p53 degradation. This study investigated the intersection of viral aetiology, p53 status, and DNA ploidy, determining if specific p53 staining patterns accurately predict HPV status, and evaluating how divergent p53 aberration mechanisms affect genomic instability.

Study design:
We evaluated 34 HPVOED specimens (from 25 patients) and 30 grade-matched non-HPVOED specimens (from 30 patients). p53 immunostaining patterns were classified as wild-type (scattered basal, patchy basal/parabasal, mid-epithelial/basal-sparing, and null-like/basal-sparing), or mutant (overexpression, null, and cytoplasmic). Ploidy was determined via image-based DNA cytometry.

Results:
Wild-type p53 patterns were more prevalent in HPVOED than non-HPVOED (91.2% vs. 43.3%, p<0.001). Mid-epithelial/basal-sparing or null-like/basal-sparing patterns strongly correlated with HPVOED (p<0.001), demonstrating 100% sensitivity, 81.1% specificity, and 89.1% overall accuracy. There was no significant difference in ploidy status between HPVOED and non-HPVOED, and ploidy demonstrated no statistical association with aberrant p53 expression.

Conclusion:
Basal-sparing p53 staining is a sensitive marker for HPVOED. Despite divergent mechanisms of p53 dysregulation, both HPVOED and non-HPVOED exhibit comparable levels of genomic instability.
