Título: CLINICOPATHOLOGICAL FACTORS ASSOCIATED WITH RECURRENCE AND SURVIVAL IN PLEOMORPHIC ADENOMA AND ITS MALIGNANT COUNTERPART

Resumo: Objective: To compare the clinicopathological features of pleomorphic adenoma (PA), recurrent pleomorphic adenoma (RPA), and carcinoma ex pleomorphic adenoma (CXPA), and to identify prognostic factors associated with survival in CXPA. Study Design: We reviewed 234 cases (148 PAs, 23 RPAs, 63 CXPAs) diagnosed between 2004 and 2024 at a Brazilian tertiary center. Comparative analyses were performed using Fisher's exact and chi-square tests; prognostic factors in CXPA were assessed by Cox proportional hazards regression for overall survival (OS) and disease-specific survival (DSS). Results: PA and RPA affected mainly women and presented as smaller tumors with shorter clinical evolution than CXPA, which arose in older patients as larger masses (all p < 0.01). Among benign tumors, recurrence was associated with a younger age at primary diagnosis (34.6 vs 43.7 years) and longer clinical evolution. In CXPA five-year OS and DSS were 53.7% and 69.2%. Symptoms, recurrence, distant metastasis, frank capsular invasion, necrosis, perineural and angiolymphatic invasion were each associated with reduced survival (p < 0.05). Conclusion: PA, RPA, and CXPA show distinct clinicopathological profiles. In CXPA, routine clinical and histological parameters carry clear prognostic weight and should guide risk stratification and follow-up.
