Título: PEDIATRIC GINGIVAL NODULE WITH MYOID DIFFERENTIATION: DIAGNOSTIC CHALLENGES AND IMMUNOHISTOCHEMICAL CONFIRMATION

Resumo: A 13-year-old male, apparently healthy, was referred. Oral examination revealed a spherical nodule on the gingival margin, at the level of the incisive papilla, between teeth 11 and 21, measuring approximately 1.1 x 1.3 x 1.0 cm. It was similar in color to the mucosa, had a smooth surface, a sessile base, and was asymptomatic. The duration of its presence was unknown. Radiographically, no erosion or bone alteration was observed. With a clinical diagnosis of fibrous hyperplasia, an excisional biopsy was performed. The tissue was sent for histopathological study, which revealed fascicles of spindle-shaped cells with eosinophilic cytoplasm, alternating with fibroconnective tissue. Once the result was obtained, authorization was requested, and the study was supplemented with an immunohistochemistry panel, where the cells showed positivity for the antibodies Vimentin, Smooth Muscle Actin, Caldesmon, and Desmin. Low proliferation index confirmed with Ki-67. The patient showed no recurrence after one year of follow-up.
