Título: CERVICAL NODAL INVOLVEMENT IN CLINICALLY NEGATIVE ORAL SQUAMOUS CELL CARCINOMA: PREDICTORS AND SURVIVAL IMPACT

Resumo: Objective: To investigate the clinical relevance of occult cervical metastases and identify predictors of nodal involvement in oral squamous cell carcinoma (OSCC) with clinically negative necks (cN0). Study design: A retrospective cohort of 74 cN0 OSCC patients treated with surgery and elective neck dissection. Variables were assessed for occult metastasis, and survival was analyzed using Kaplan-Meier and Cox models. Results: Occult metastases were identified in 20.3% of cases, with higher frequency in tumors of the floor of the mouth. Lymphovascular invasion (LVI) significantly increased the likelihood of nodal involvement (OR = 4.62; p = 0.043). Survival analysis showed that both occult metastasis and LVI were independently associated with poorer outcomes. Patients with occult metastases showed reduced disease-specific (HR = 4.66; p = 0.021) and overall survival (HR = 3.77; p = 0.024). Inclusion of level IV in neck dissection did not improve detection of occult disease or survival outcomes. Conclusion: Occult metastases significantly impact prognosis in cN0 OSCC. LVI may help identify high-risk patients, while extension of neck dissection to level IV appears to offer no additional clinical benefit.
