Título: DOSIMETRIC ANALYSIS IN MICROVASCULAR FLAP RECONSTRUCTIONS IN HEAD AND NECK CANCER PATIENTS SUBMITTED TO RADIOTHERAPY

Resumo: OBJECTIVE:

To evaluate the incidence of complications following reconstruction with anterolateral thigh (ALT) or fibular flap (FF) and their correlation with radiotherapy (RT) doses delivered using intensity-modulated radiotherapy (IMRT).

STUDY DESIGN:

This observational retrospective cohort study included patients who underwent reconstruction with ALT or FF, with or without adjuvant RT. Patients were divided into three groups: Group A (ALT with RT), Group B (FF with RT), and Group C (ALT or FF without RT). Demographic and clinical variables were analyzed.

RESULTS:

A total of 72 patients were included (median RT dose: 62 Gy; range: 18.2-76.4 Gy), with predominance of male smokers. Group A retrieved 41 patients (56.9%), Group B 17 (23.6%), and Group C 14 (19.4%). Overall complications included plate exposure (12%), fistula (20%), infection (16%), and osteoradionecrosis (24%). In Group A, complications included fistula (12.5%), infection (25%), osteoradionecrosis (25%), and other complications (37.5%). In Group B, plate exposure, fistula, and osteoradionecrosis each occurred in 33.3% of cases. In Group C, complications occurred in 33.3% of patients.

CONCLUSION:

RT doses between 21 Gy and 67 Gy were not associated with increased incidence of acute or late complications in patients undergoing reconstruction with ALT or FF.
