Resumo: Ameloblastic fibroma (AF) is a rare benign mixed odontogenic tumor composed of odontogenic epithelium and mesenchymal tissue without dental hard tissue formation. A 25-year-old, caucasian woman presented with pain in the left mandibular region for one year. Clinical examination revealed normal mucosa without evident alterations. Imaging showed an intraosseous, unilocular radiolucent lesion in the periapical region of teeth 37 and 38, measuring approximately 45 mm. The lesion was totally removed under local anesthesia. Histopathological analysis revealed an odontogenic neoplasm composed by strands and islands of odontogenic epithelial cells in a loose connective tissue stroma resembling primitive dental papilla. The islands of odontogenic epithelium presented the peripheral cells resembling ameloblasts and the central cells were loosely arranged, resembling the stellate reticulum of the enamel organ. The strands or cords sometimes were long, often anastomosing. Based on histopathological findings, a diagnosis of ameloblastic fibroma was established. The patient remains under periodic follow-up.
